If your insurance company does not pay within 60 days, you will be responsible for payment of all charges.
If your insurance plan determines a service is not covered, you will be responsible for the complete charge. Should this occur, we will bill you. The payment is due upon receipt of that statement.
Your dental insurance is a contract, which you and your employer have arranged with one of many carriers. We do not have a direct relationship with these carriers. We are not contracted to participate in any dental networks. We do not participate in the Medicaid Managed Care plans.

Please bring new insurance cards to your next office visit.
Before coming to our office, please make sure all new insurance cards are correct. Check all names and numbers carefully. Call your insurance company if anything is incorrect. Please make any corrections before your appointment to avoid insurance rejections.
